Services Alzheimer's and Dementia Specialty Team
The goal of the Alzheimer’s and Dementia Specialty Team to ensure patients with Alzheimer’s disease receive the most appropriate care is achieved by educating and training staff, families and caregivers. All disciplines at Norwell VNA and Hospice understand and are taught the most current treatment and interventional approaches for optimal patient disease management. Working one on one with both the patient and caregivers, they advise and guide them on how to remain safe and as independent as possible in the home.
A number of our home health care professionals have received advanced training to improve their skills and ability to provide services to these special patients and their families.
Care Team
- An interdisciplinary Alzheimer’s team utilizing integrated disease assessment and management strategies.
- Team consists of experienced nurses, physical therapists, occupational therapists, and geriatric care manager.

- Long-term planning
Community Services Provided
Monthly Caregivers support group provides resources and assistance to caregivers of individuals in the mid to late stages of Alzheimer’s disease.
Educational program and support group provides information, resources and assistance to individuals with early stage Alzheimer’s and their care partners.
Lifeline coordinated through Norwell VNA and Hospice to help provide peace of mind when a person is at home alone.
