Services Diabetes Disease Management Specialty Team
Diabetes is one of the top diagnoses serviced by the Norwell VNA and Hospice. This specialty team provides fellow clinicians, patients and families the most up to date education and self-management approaches to manage this chronic illness.
Care Team
- An interdisciplinary diabetic team
- Team consists of Certified Diabetic Educator, experienced diabetic home care nurses, dietitian, physical therapists, and occupational therapists.
Services Provided
- Evidence based diabetic care pathways
- Diabetes educator assures individualized assessments for patients diagnosed with diabetes.
- Each assessment is based on patient specific goals. The assessment covers monitoring glucose, target glucose, and A1C levels, decreasing long-term risks and complications, nutrition strategies, foot care, community services, insulin preparation and administration, and the effects of medications on glucose levels
- Intensive patient/caregiver family education
- Telemonitoring available to assist in tracking blood glucose levels to optimize patient self management and education
- Laboratory services
Community Services Provided
On-going free Monthly Diabetic Support Group:
Individuals with diabetes can offer each other support and information at our diabetes support group. This group allows participants to share their experiences with diabetes, learn from others, and interactively discuss how they live with diabetes. Family members and caregivers are welcome. There is no fee for this group.
On-going American Diabetic Association and Medicare Certified Diabetic Self Management Education Program:
This referral-based program provides diabetes education for each client. Clients meet with a registered dietician or certified diabetes educator for an individual assessment.
Group classes consist of a series of four sessions that cover all aspects of diabetes management, from basic information to advanced level skill.
Fees for this program are covered by most insurance plans; a written referral or script is required. If you wish, please contact your insurer about coverage and out-of-pocket expenses. - Diabetes Overview/Medications
- Nutrition: Meal planning, carbohydrate counting
- Exercise and Monitoring your blood sugar
