Geriatric Case Management

Norwell VNA and Hospice was founded on the principle that home health care provides a cost-effective, clinically sophisticated option for patients who wish to remain in their homes. This couldn’t be more true today with shortened hospital stays and rising healthcare costs. We provide many of the same services and professionals available in hospitals and rehabilitation facilities, including:

• Nurses and Nurse Practitioners
• Rehab specialists in physical therapy, occupational therapy, and speech therapy
• Medical Social Workers
• Home Health Aides

When a Geriatric Care Manager is needed to coordinate and oversee all care, our private duty division, NVNA Works, offers such professional services.
