Overview of GCM
Our Geriatric Case Management Services start with a Geriatric Care Manager who works with the client and family, if appropriate, to assess and evaluate the client’s needs. Coordinated through our private duty division, NVNA Works, our Geriatric Care Manager offers clients personalized services including:
- Medical assessment and education
- Medication management
- Medical appointment coordination
- Coordination of professional services including nursing, physical and occupational therapy, nutrition, and social work
- Referral to para-professional services including home health aide, personal care aide, and companion care.
- Links to community and professional resources
- Liaison for family members and/or others involved in clients’ care.
Medicare does not cover Geriatric Case Management Services. However, many long-term care insurance policies will pay for such services. Our Geriatric Care Managers will assist in reimbursement from long-term care insurance, if applicable.
