Hospice Clinical Services
Norwell VNA and Hospice has a team of medical professionals that collaborates to meet the specific needs of each of our patients.
Our Medical Director works alongside our patients’ Attending Physician and assists in assuring the patient’s comfort and symptom management.
Dr Gail Gazelle, MD, FACP, FAAHPM has devoted her medical practice to end of life care. In addition to working with patients, Dr. Gazelle is also actively involved in teaching doctors about hospice. She is an Assistant Clinical Professor at Harvard and a master facilitator with Northwestern University teaching doctors around the country the range of skills needed to competently care for people at end of life. Our Nurses are specifically trained in pain and symptom management. Each patient has a nurse case manager who provides care and monitors the patient’s condition through home visits and consultations with other medical professionals. The nurse also teaches the family and caregivers how to care for their loved one. Patients and families are encouraged to call the NVNA Hospice office with questions or concerns whenever they arise. NVNA Hospice also provides 24-hour clinical on-call availability.
Our Social Workers provide counseling, grief support, and assistance with accessing community resources. The social worker meets with both the patient and the family as needed and requested.
Our Chaplain respects each patient's personal beliefs and supports individual spiritual needs.
Our Home Health Aides are also specially trained in hospice care. They provide assistance with personal care, including bathing, toileting, dressing, and skin care to keep the patient comfortable.
Our Therapists, which includes physical, occupational and speech therapy, can step in to address safety issues, as needed. |
