Benefit Summary (2015)

Health Insurance - Harvard Pilgrim Healthcare HMO
Participation requires benefit eligible status and a minimum work schedule of 30 hours per week. Different coverage levels and plan designs are available.

Dental Insurance
Participation requires benefit eligible status and a minimum of 20 hours per week. Different coverage levels and plan designs are available.

125 Flex Spending Plan
Medical Reimbursement and Dependent Day Care.

Life, Short-Term and Long-Term Disability Insurances
Provided free of charge to employees who meet eligibility criteria.

Life Insurance: 1x annual salary

Short-Term Disability: 7 day waiting period then benefit is equal to 66 2/3% weekly salary

Long-Term Disability: After short-term is exhausted, pays 60% of monthly salary to a $4,000 max.

Earned Time Program
Combines vacation, holiday, personal and sick time into one bank. Accrual starts at eligibility and rates are based on hours worked and years of service.

401(k) Retirement/Pension Plan
All employees age 21+ are eligible. Discretionary match up to 6% annually. 100% vested after 3 years of service.

Educational Assistance
Up to $2000 per year for benefit eligible staff depending upon hours worked.
