Job Description:

Part-time (24 hours)

The Palliative Care NP will provide advanced level of nursing practice servicing as a clinical resources and consultant to the clinical staff in developing and implementing the plan of care through the application of the nursing process and in accordance with the philosophy, mission and standards established by the Agency.  Performs independent visits and/or co-visits to assure patients with palliative care needs are being managed according to best practice standards.  Performs initial admission and/or evaluations of patients requiring palliative care, maintains records and reports and meets electronic documentation requirements in a timely fashion. 


Preferred Skills:


  • Proven ability to instruct/teach patients, family members, clinicians.
  • Excellent assessment skills
  • Good understanding of Medicare Conditions of Participation for home care/hospice
  • Good computer skills; knowledge of electronic medical records
  • Able to work independently with minimum supervision
  • Excellent communication and organizational skills required
  • Able to multi-task; detail oriented and flexible 
  • Current MA license to practice (in good standing);
  • BSN required; Advanced Practice Nurse required
  • Current CPR certification required
  • Current Specialty Certification required
  • A minimum of five years of direct patient care experience required (acute    care setting)
  • Previous community/home health experience preferred
  • Practice experience in internal medicine, geriatrics, or oncology preferred (experience appropriate to very complex case management with issues of aging.)
