Dear Friend, 


Managing healthcare decisions for a loved one is not an easy task. Whether it is choosing an assisted living facility, finding the right services for memory-impaired individuals, or interpreting insurance coverage, my 35 years of experience as a nurse have shown me that there is no one clear-cut answer for all. Every situation is different. It is my goal to help you smoothly find your way through. 


Navigator 27 is a consultative division of NVNA and Hospice that assists clients and their families as they traverse the challenges of aging and care transitions. 


We are here to help ensure that both you and your loved ones benefit from the support needed to live life to the fullest in the best care setting. 


Whatever your personal needs and concerns, my team and I will work with you to address them. In addition to acting as a virtual support system, we can also join you on visits to assisted living and skilled nursing facilities or accompany family members to discharge planning meetings at hospitals and acute care facilities.

How Can Our Concierge Care Help You and Your Family?

As a leader in home health care and hospice, we have over 100 years of clinical expertise, health care experience, knowledge, insights, and care connections. There are many ways Navigator 27 can be of service to you. For example, we can:

  • Advise you on how to manage and coordinate short and long-term care plans for you or your loved one

  • Help you to interpret and maximize insurance coverage options including Medicare and private health insurance plans

  • Identify appropriate independent living and assisted living facilities and help you to know when it’s time to transition

  • Schedule and accompany you to appointments to visit and evaluate facilities

  • Assist you with difficult or emotional decisions that are sometimes complicated by conflicting family opinions

  • Offer specific guidance regarding memory-impairment and services available

  • Give objective, skilled assessments of your current care needs along with referrals to home care services including certified home care, palliative care, hospice care, private duty assistance or other appropriate community resources

  • Attend discharge planning meetings at acute care hospitals and skilled  nursing facilities, participating in pivotal medical appointments if needed
